Sorry i have no way of knowing what actually happened and prefer not to comment when i have limited knowledge of a situation.

And as you so clearly said it's usually not the sin that one is disfellowshipped by, it's their attitude and either misunderstanding of GOD'S law on it and how they react and if they are repentant.
That is why which has been expliained to them CLEARLY that there can be multipul meetings with that person for many reasons and one is to "not just disfellowship them on the spot".
There is always enough effort made by the elders to help that person by scripture which is from GOD and then give them time to see how that person reacts.
If there is no repentance and if danger to the congregation is apparent, then they could be disfellowshipped.
As you mentioned this is ALL in the bible, for all to see. It is obvious they CAN'T see it and that is very sad as God's laws are perfect and just.
